May 27, 2009 - 9:44am - December 30, 2011 - 3:45pm

The New Access to Insurance Program is now offering application assistance for Family Medicaid and CHP+ Child Health Insurance. The Denver Children's Advocacy Center is assisting families in the 7 surrounding Denver Metro counties with enrollment every weekday from 9:00 a.m. to 5:00 p.m.

Make your appointment today! Each appointment is only one hour and families will be provided with case management to help ensure they stay enrolled. For the appointments families need to bring:

  • Each child’s birth certificate
  • If wanting to enroll the adults in the household, a Driver’s License or State ID for each adult wanting to enroll and his/her birth certificate
  • One month of pay stubs from either the current or prior month
  • If unemployed, their unemployment benefits letter stating their weekly amount paid if receiving, otherwise no income can be stated

For assistance with obtaining birth certificates, we provide the family maps and information about the Colorado Vital Records offices.
